<p>Issues: We present key aspects of our paper, commissioned by UNAIDS in 2005, entitled, “Revisiting the ethics of HIV prevention research in developing countries.” In 2004 and 2005 we witnessed the closure or suspension of three international clinical trials testing tenofovir in the prevention of HIV infection in high risk groups due to the failure to provide free treatment to those who seroconvert during the conduct of the study. We examine critically moral claims for the provision of treatment to those who seroconvert in HIV prevention trials and ask whether it is a matter of moral obligation or moral negotiation? Description: Using the tools of philosophical and regulatory analysis we examine the moral foundations of research ethics, international regulation, and the contemporary literature in search of valid arguments in support of a moral obligation to treat. Arguments supporting the obligation to provide such treatment are found in a wide variety of documents. These claims are variously rooted in the demand to compensate for research-related injury, maximization of research benefit, justice as reciprocity, and justice as equality. Lessons learned: Our analysis reveals that there is as of yet no robust moral argument supporting a moral obligation to provide treatment to participants in HIV prevention trials who seroconvert. We argue that moral negotiation offers distinct advantages over moral obligation. Moral negotiation finds support in the principle of respect for communities that holds researchers responsible to respect communal values, protect and empower social institutions and abide by the decisions of accepted communal authorities. Recommendations: Moral negotiation, rather than moral obligation, reduces the chance of polarization of positions by inviting trial participants, sponsors, researchers and all other involved parties to the table for discussion. Moral negotiation favors middle ground solutions that fit the specific health needs and priorities of the community in question.</p>

Primary care physicians and emergency dept. physicians are often called upon by patients to determine whether individual patients should be prescribed adrenaline auto-injectors (AAI’s). Although these patients are often also referred to an allergist to help determine whether their condition warrants this, the wait may often be for months. We set out to determine how accurate referring physicians are in determining that patients should be prescribed AAI’s.

Stachybotrys atra has achieved great notoriety recently as a mould capable of producing mycotoxin, a potentially quite harmful substance. Because of news reports, patients have become quite concerned about “mould allergy” as the cause of an increasing number of symptoms. We set out to discover what percentage of patients referred to regional Allergy clinics have become sensitized to moulds, but especially Stachybotrys atra.

<p>Background</p>
<p>The prevalence of peanut allergy is increasing. The reasons for this are not entirely known. A factor may be vitamin D (Vit D).</p>
<p>Methods</p>
<p>This study was performed in a referral allergist’s office in Ontario. Prospectively, all patients (<18 years old) with peanut allergy who were tested for peanut specific IgE (PN IgE) also had Vit D measured. All measurements were done between December 2010 and May 2011. The Vit D measure was 25-hydroxy vitamin D. Patients were divided into three groups: deficient (less than 25 nmol/L), insufficient (25-75 nmol/L) and sufficient (75-250 nmol/L). Vit D levels were compared to PN IgE, sex, age, body mass index (BMI) and other allergies.</p>
<p>Results</p>
<p>Fifty peanut allergic patients were included. The mean Vit D level of the patients was 73.8 nmol/L and the 95% confidence interval was 69.6 - 75.7 nmol/L. One patient (2%) had deficient and thirty-one (62%) of the patients had insufficient Vit D levels. Nineteen (38%) had Vit D levels in the sufficient range. There was no correlation between Vit D levels and PN IgE or BMI. Generalized linear modeling showed that vit D levels were predicted by age and sex (p=0.04 & p=0.002, respectively).</p>
<p>Conclusions</p>
<p>Two percent of our patients had deficient Vit D levels while 62% of our patients had insufficient Vit D levels. These levels were statistically associated with age and sex. Insufficiency of Vit D may play a role in peanut allergy.</p>
